Online bookings let you set a default patient form to automatically send after patients book appointments. You can also assign a different form to specific fees or even opt to send forms only for these specific fees. This allows you to customize the information you request from patients, depending on the fee in their booked appointment.
You can also choose a patient form that Halaxy emails automatically when you manually create an appointment that includes that fee.
Important
To use online booking patient forms, you must first enable both online bookings and automatic patient forms for online bookings.
Note
If an appointment type has its own patient form, Halaxy sends that form instead of the linked fee's. To send the fee's form, remove the form from the appointment type or unlink the fee.
Use this option to send a patient form automatically after a patient books online. The patient receives a link to the form.

Click Finances > Fees.

Click the fee from the list.

Under Fee Details, for Online booking patient form, select the form template to link to this fee.

Click Save Fee.
When a patient books this fee online, they receive a link to the linked patient form.
To remove the linked form template, for Online booking patient form, select Do not send a patient form or Use group default.
Use this option to send a patient form when you create an appointment in your calendar. Halaxy emails the form to the patient.

Click Finances > Fees.

Click the fee from the list.

Under Fee Details, for Manually created appointment patient form, select the patient form template to send.

Click Save Fee.
When you create an appointment that includes this fee, Halaxy emails the patient the form.
To remove the linked form template, for Manually created appointment patient form, select Do not send a patient form.
If an appointment has several fees, Halaxy sends the form from the first fee that has one, including linked fees. If the appointment type has its own form, Halaxy sends that form instead.
Important
Halaxy sends this form only when automatic patient forms are turned on in your patient form settings.
Note
Halaxy doesn't send the form for online bookings, past appointments, or patients without an email address or who have opted out of email.
